Fragments are little nuggets of information like frozen shards of time. You just have to touch them to fill your head with what's inside, kupo. You see, they are made of the same crystal that's inside the Oracle Drive. That's how they hold memories and stories and so much more.
Mog in Spinning Moonstone Fragment
Fragments are items displaced by paradoxes, a means of integrating the in-game menu into the storytelling process, and gameplay objectives in Final Fantasy XIII-2. They can be obtained by various means: progressing through the storyline, winning a certain battle, unveiling them with Mog'sMoogle Hunt ability from where they are hidden by time paradox, or through quests. The fragments are inscribed with tidbits of knowledge, folklore, and legends which can be viewed in the Fragment section of the menu. There are 160 fragments to collect and there are several held in each time period.
Fragments are organized two ways, each with a set of sub-categories. Locations is a set organized by location, and thereby listing the missions in that location, which is more of an organization by a game mechanic. The Categories set is organized either by the source of the fragment texts, as with Great Mog Wisdom and the diaries from Lenora's Garage, or by their subject, as with the paradox ending texts. When specific sets of fragments are collected, Fragment Skills, such as Paradox Scope, can be unlocked by the Mystic in Serendipity.
Achievements can be unlocked in this way, as well as new fragment missions. For example, the Crimson Memories fragment set, when collected, can unlock the Fragmented trophy/achievement and automatically creates a time distortion and a cutscene leading to the mission to kill Ugallu and gain its fragment. Collecting them can be a source of artefacts, which then can unlock more fragments. As an example, the full set of Graviton Core fragments are required to obtain the Vagabond Artefact and its accompanying paradox ending, itself a source of another fragment.
Collecting all 160 fragments earns the Defragmented trophy/achievement.
Find the key on Paradox Alpha's hand with Moogle Throw and talk to the guard Morris, by the cell. Unlock the gate in the cell and open the "out-of-phase" treasure sphere in the tunnel with Moogle Hunt.
Speak to Ronan in the Lamentable Rest area, then examine the carbuncle doll by the gravestone in Bresha Ruins -300 AF- (available after completing the Palladium, Osmium, and Rhuthenium Ring quests).
Obtain the Adamantite Ring sidequest in Bresha Ruins -100 AF- and then speak to the Government Agent by the Atlas Control Device and solve the Temporal Rift.
Obtain the Sealed Tablet located in the treasure box below the ramp leading up to the giant head in the Paddraean Archaeopolis and return it to the Ruby of Grief.
Start the Adamantite Ring sidequest in Bresha Ruins -100 AF- and obtain the Thorne's Information key item from Uma in Yaschas Massif -100 AF-. Talk to the "out-of-phase" man in dead end of the Pass of Paddra and answer "Thorne".
Interact with the computer in northwest room on 50th Floor, speak to Alyssa'sduplicate, then use Moogle Throw in the room with Chocolina to get Paradox Agent Type A.
Use Moogle Throw to reach the treasure box containing [List of Final Fantasy XIII-2 items#Paradox Agent Type B|Paradox Agent Type B]] on 49th Floor, then interact with the computer on 50th Floor (complete Idea Circuit first).
On the outer walkway of the 49th Floor, use Moogle Throw to reach the treasure box hidden below the edge containing Paradox Agent Type C, then examine the computer on 50th Floor (first complete Enigma Codex).
Close the Academia -4XX AF- gate and deactivate the Paradox Scope. Refuse Lightning's offer in New Bodhum, reactivate Paradox Scope, continue to A Dying World, and fight Caius.
Fragment Skills are additional skills you can unlock to enhance your characters and overall gameplay experience. After completing the required objectives, go to Serendipity and speak with the Mystic in the building tucked away in the southwest corner of the map.

Allows the player to increase or decrease the enemy spawn rate on the field. This does not affect the enemy spawn rate in Academia 500AF.
Obtain all fragments from the Yaschas Massif in all time periods.
Eyes of the Goddess
Allows the player to assume a degree of control of the game camera during certain cutscenes.
Obtain all fragments from New Bodhum -003 AF-, Bresha Ruins -005 AF-, Yaschas Massif -010 AF- and -01X AF-, Oerba -200 AF- and Sunleth Waterscape -300 AF- (excluding paradox endings).
Field Killer
Allows the player to defeat most enemies instantly by attacking them when they spawn in the field, but doing so gives no items, monster crystals, Gil or CP.
Obtain all fragments from the Academic Reward and Great Mog Wisdom categories.
