The Death Eye, also known as the PHANTOM in the NES version, is an enemy from the original Final Fantasy. It is a fixed encounter the party must face before they can access the lower levels of the Chaos Shrine. On versions where encounters were not removed from Chaos Shrine B5 (e.g., Pixel Remaster, original JP Famicom), it may also be found as a random encounter on that floor only. Starting from Dawn of Souls, the Death Eye has a 1% chance of dropping an Elixir.

Due to its low HP, the Death Eye is typically not a threat. However, it can break resistances via Dispel, then follow up with several instant death type attacks. Using Diaga or Firaga is recommended to dispose of it quickly.

Much like the Evil Eye, in the game's original Japanese release the Death Eye, then named Death Beholder (デスビホルダー, Desu Behorudā?), so resembled the Beholders from Dungeons & Dragons that Nintendo, who distributed the NES version of the original Final Fantasy, redesigned the shared sprite between the two monsters before bringing the game to the US.
